 The portal vascular resistance was measured percutaneously in 28 patients with chronic liver diseases. The portal vascular resistance (PVR) was calculated from the portal blood flow (PBF), the portal venous pressure (PVP), and the hepatic venous pressure (HVP), all of which were measured simultaneously. The PVR was calculated by the equation, PVR=(PVP-HVP)/PBF. The PBF was measured by an ultrasonic duplex system composed of a real-time B-mode scanner and a pulsed Doppler flowmeter. The PBF was calculated from the cross-sectional area measured by the B-mode and the average blood flow velocity measured by the Doppler mode. The PVP was measured by percutaneous transhepatic catheterization under ultrasonic guidance and the HVP was measured by venous catheterization. The results are as follows; The PVP was 17.6±3.9 mmHg (mean±S.D.) in patients with chronic active hepatitis (CAH, n=10) and 20.3±2.4 mmHg in cirrhosis (LC, n=18). The PBF was 973±395 ml/min in CAH and 778±272 ml/min in LC. The PVR was 0.68±0.33 mmHg・ml-1・min・kgBW in CAH and 1.25±0.73 mmHg・ml-1・min・kgBW in LC. In the PVR, there was a significant difference between CAH and LC.
