A case of bilateral and multiple renal angiomyolipoma without tuberous sclerosis was reported. There were multiple marked hyperechoic nodules in bilateral kidney and a large hypoechoic mass at the lower pole of the left kidney. We considered the nodules as multiple angiomyolipoma and the mass as renal cell carcinoma. But pathologically, both were angiomyolipoma and the former were mainly composed of lipoma and the latter was mainly composed of myoma. Angiomyolipoma has come to be easy to diagnose by CT, Ultrasound and so on, but hypoechoic angiomyolipoma lacking in adipose cells is still apt to be confused with renal cell carcinoma. We realized keenly that needle biopsy or aspiration cytology should be done in such a case.
