We recently experienced two cases of coronary artery fistula in childhood with successful repair in surgical operation. In these cases, real-time two-dimensional Doppler (2D-Doppler) was useful for diagnosis. In our experience, the findings of coronary fistula in 2D-Doppler were as follows; 1) rapid blood flow pattern in diastolic phase, in the dilatated coronary artery (coronary aneurysm) and ascending aorta. 2) blood flow pattern at the outlet orifice of fistula in the heart cavity. However, these findings changed according to the volume of shunt flow, so for accurate decision of the site of orifice or detection of small shunt flow, confirmation by angiographic approach is necessary.
