Percutaneous Transhepatic Ultrasonic Lithotripsy in the Treatment of Biliary Stones

With a Wolf ultrasonic lithotriptor and rigid nephroscope, we had applied percutaneous approach under echo-guidance to gall bladder, intrahepatic duct and common hepatic duct stones. Disintegration of gall stone with sonotrode was performed in 11 patients of gall stones, 3 of intrahepatic stones and 2 of common bile duct stones. There were 3 failures in percutaneous transhepatic cholecystoscopic ultrasonic lithotripsy (PTCCS-USL), but all were successful in the percutaneous transhepatic cholangioscopic ultrasonic lithotripsy (PTCS-USL) group. Immediate cholecystectomy was performed in the failure cases. There was no severe bleeding nor bile leakage. In conclusion, ultrasonic lithotripsy might play an important role in the future treatment of high risk group of biliary stones.
