A 62-year-old male was pointed out a hypoechoic mass (15×13 mm) with unclear margin in the liver with ultrasound. Hepatocellular carcinoma was suspected and aspiration cytology was performed under real-time ultrasonic guidance. The cytology revealed class-2. Next morning, he complained epigastralgia and 3 days after ultrasonically guided puncture, a lot of tarry stool was discharged. Next day, ultrasonic examination was performed and an echogenic mass with strong echoes in the gallbladder was demonstrated. We concluded that transient hemobilia was due to the ultrasonically guided fine needle aspiration biopsy. Up to now, no serious complication during and after fine needle aspiration cytology was reported. However, we should be careful about a hemobilia after the fine needle aspiration cytology.
