Diastolic filling of the right ventricle in diabetes mellitus was evaluated from the right ventricular inflow pattern using pulsed Doppler echocardiography and was compared with abnormalities of the left ventricular inflow dynamics. The study comprised 40 cases with diabetes mellitus and 20 normal subjects. The diabetics was divided into two groups according to the presence or absence of microangiopathy (Group I: 20 cases without microangiopathy, Group II: 20 cases with microangiopathy). Microangiopathy is defined as the presence of diabetic retinopathy and/or persistent proteinuria. The patients had no hypertension. The right and left ventricular inflow patterns were recorded from the parasternal and apical approaches. Acceleration rate, deceleration rate, peak velocity in the rapid filling phase (R), that in the atrial contraction phase (A) and the ratio of A/R were measured in the biventricular inflow patterns. (1) Acceleration rate and deceleration rate of the right ventricular inflow velocity pattern in diabetics (Group I: 337±85 cm/sec2 (ns), 191±35 cm/sec2 (p2 (p2 (p2, 239±70 cm/sec2). The ratio of A/R significantly increased in the diabetics (Group I: 0.95±0.16 (p
