Twisted Parovarian Cysts

Torsion of a parovarian cyst is uncommon, presenting as an acute abdomen and commonly misdiagnosed as the torsion of other adnexal masses, an ovarian accident, appendicitis, etc. Recently, we experienced two cases with twisted parovarian cysts. In one case, it was associated with 21 weeks' intrauterine gestation. In this case, pelvic sonography during the first trimester showed no cystic lesions. In the other case, a fever and lower abdominal pain continued after appendectomy. A sonogram revealed a large cyst resembling a distended urinary bladder. These twisted parovarian cysts were removed at laparotomy. The clinical and sonographic features of the twisted parovarian cysts are briefly discussed and literatures are reviewed.
