A 67 year-old female was admitted to our hospital for the evaluation of liver tumor. A ultrasonographic study on admission showed a cystic mass behind the pancreatic head, freely communicating with the lumens of the portal, superior mesenteric, and splenic vein, which was diagnostic of portal vein aneurysm. This diagnosis was confirmed by abdominal CT scan and portography. Hepatocellular carcinoma associated with liver cirrhosis was also found. Follow up study revealed no remarkable alteration of this aneurysm by ultrasonography. After 13 months she died of hepatic failure.
