We present a case of carcinoma of the hilar bile ducts in which a dilated ductal branch within the caudate lobe (B1) was successfully visualized with ultrasonography. The visualization was accomplished with the PTCD catheter as a guide for tracing, which had been inserted into B1 via the intrahepatic bile duct of the superior posterior segment. This bile duct (B1) was also detected by an intraoperative scanning over the surface of the caudate lobe.
Contrary to a general belief of difficult visualization of the B1 with ultrasonography, our experience suggests a possibility and usefulness to identify not only the B1 itself but also an invasive spread to the B1 from a cancer of the hilar bile ducts.
