大きな嚢胞像を認めた大腸癌肝転移の1例

A 73-year-old woman was admitted to our hospital presenting a large abdominal mass. Laboratry findings on admission revealed elevated serum LDH (523 mU/ml), CEA (256.5 ng/ml) and CA19-9 (164 u/ml) levels. Ba-enema showed a large crater in the ascending colon and ultrasonogram demonstrated four large cystic lesions in the liver each measuring about 8~10 cm in diameter. CT with contrast enhancement showed almost identical findings as US. Stretching and displacement of arteries were visualized on angiographic examination. Resection of the ascending colon, right lobe and S3 segment of the liver was performed. Histpathological examination of the resected colon and liver showed well differentiated adenocarcinoma.
Although small anechoic zones within metastases are occasionally seen, metastases with large sonolucent components like this case are rare.
