We recently encountered a patient with Lesch-Nyhan syndrome. The ultrasonographic images of both kidneys were simillar to those found in patients with gout. The patient was a 9-year-old boy who had been treated for cerebral palsy with hyperuricemia and other neurological disorders (athethosis and rigidity). The US images of both kidneys revealed hyperechoic medulla with no acoustic shadowing. The hyperechoic medulla was believed to represent deposits of either uric acid or xanthine crystals along collecting tubules and secondary interstitial fibrosis.
