A case of an aortic root abscess without vegetations complicating infective endocarditis was examined using a two-dimensional echocardiography. A 61-year-old woman was admitted to our hospital with fever and general fatigue. Upon admission, a grade IV systolic murmur and grade II diastolic murmur were audible at the base. Therefore, a two-dimensional echocardiography was performed. We suspected infective endocarditis, but because she had neither a prosthetic valve, a congenital bicuspid valve of aorta, nor vegetations, it was impossible to confirm infective endocarditis. But our careful repeating of two-dimensional echocardiographic examinations did finally confirm infective endocarditis by detecting an increasing echo-free cavity prior to surgery. We conclude that careful repeating of two-dimensional echocardiographic examinations for detecting a valve ring abscess make it possible to diagnosis infective endocarditis early and to decide the indication for surgery, even if vegetations can not be datected.
