Contraction Ultrasonography for Gallbladder Diseases

We designed a new diagnostic method for gallbladder (GB) diseases using contraction ultrasonography (US). The usefulness of this method was evaluated. Eighty–one cases with GB diseases consisting of 62 cases with polyp, 8 with adenomyomatosis, 3 with cancer, 2 with cholecystolithiasis and 6 with other GB lesions, were examined by contraction US. GB was scanned by convex transducer (3.75 MHz or 3.5 MHz) before and after contraction with intramuscular injection of ceruletide diethylamine (ceosunin, 0.2 μg⁄kg). Ultrasonographic findings of GB (visualization of each part of GB, three–layer structure of GB wall and echoic image of GB lesions) after contraction were compared with those before contraction. Contraction US was superior to conventional US in visualization of the fundus of GB. Three–layer structure of GB wall was well recognized in 72% by contraction US, whose rate was higher before contraction. This structure was easily identified when GB was strongly contracting. Echoic image of GB lesions were more clearly demonstrated in 51% after contraction.
These results suggest that contraction US was useful for the diagnosis of GB diseases, because this procedure provided us clearer visualization of whole image of GB, three–layer structure of GB wall, and GB lesions.
