Using a real time ultrasonography, 16 patients with acute gastric mucosal lesion (AGML) and 9 patients with gastric anisakiasis were examined. Definitive diagnoses were made by gastric endoscopy. Thickening of the gastric wall, mainly of the 3rd layer, was detected in each disease. Each group was evaluated for gastric wall thickness and the echogenicity of the gastric 3rd layer. For the assessment of the echogenicity of the gastric 3rd layer, echo levels were classified, as adovocated by Kajiyama et al. In gastric anisakiasis, gastric wall thickening was more marked than that in AGML. Also, the 3rd layer of the gastric wall in gastric anisakiasis was less echogenic than in AGML. This result suggests that ultrasonography may be useful for the differing diagnoses between AGML and gastric anisakiasis.
