Hemangioma of the spleen is rare but is the most common primary neoplasm of the spleen. We had 5 patients with splenic hemangiomas and performed ultrasonic examinations before surgery. Three of our patients had solitary hemangioma of the spleen and two had multiple hemangioma. Echo patterns of splenic hemangioma were classified into two different types: a hyperechoic pattern and a complex pattern.
We studied the correlation between ultrasonograms and pathologic findings. Hemangioma tissue without degeneration or necrosis was found in the ultrasonic hyperechoic area. This suggests that hypoechoic areas reflect necrosis, fibrosis, bleeding and dilated vessels in the tumor and anechoic areas reflect cysts in the tumor. We believe that marginal fibrous tissue, such as the capsule, corresponded to the marginal hyperechoic or hypoechoic zone on ultrasonograms.
