A 58-year-old woman was admitted to our hospital with liver dysfunction. Laboratory data was positive for anti-mitocondrial antibodies. An abdominal echogram showed a hypoechoic area in the left lateral segment of the liver, which communicates the portal vein with the hepatic vein. Color doppler images showed clearly that two color blood flow is mixed in that area. Therefore, we made a diagnosis of portal-hepato venous shunt (PV shunt) to the abnormal area, and confirmed it with angiography. On the other hand, liver biopsy showed early stage of primary biliary cirrhosis (PBC). Recently there have been an increasing number of reported cases of PV shunts, but we have never seen PV shunt cmplicated with PBC in Japan, and color doppler was very useful in diagnosing this case.
