A 60-year-old man with coronary artery fistula communicating with the left ventricle was reported. The initial clinical impression was aortic valve regurgitation, because of diastolic blowing murmur. But a dilated right coronary artery was visualized through two-dimensional echocardiography, and an abnormal mosaic pattern in the left ventricle during diastole was visualized in Doppler color flow imaging. These findings were strongly suggestive of a right coronary artery fistula draining to the left ventricle. The diagnosis was confirmed by coronary arteriography. Doppler color flow imaging is very useful in diagnosing of coronary artery fistula.
