To investigate the serial changes in left ventricular diastolic filling after percutaneous transluminal coronary angioplasty(PTCA) , isometric handgrip exercise(IHG)•Doppler echocardiography was performed before and after PTCA(3 days, 1 and 3 months, 1 year) in 8 patients with effort angina pectoris(AP). All patients had one vessel disease and had no re-stenosis documented by coronary angiography on 3 months and 1 year after PTCA. Peak rapid filling velocity(R), peak atrial filling velocity(A), and the ratio of peak atrial filling to peak rapid filling velocities(A/R) were measured from the transmitral flow velocity pattern at rest and during IHG. Before PTCA, A/R at rest was significantly higher(1.33±0.12 vs 0.92±0.04, p These results indicate that subclinical impairment of LV diastolic filling lasts for at least 1 year after PTCA, and prolonged impairment of LV diastolic filling after PTCA is likely to occur due to a hibernating myocardium caused by recurrent anginal attack.
