Usefulness of percutaneous ethanol injection therapy (PEIT) is widely accepted in the treatment of hepatocellular carcinoma (HCC). However, complications due to misdirection of ethanol from the lesion into the peritoneal cavity and circulation have been reported. In an attempt to reduce the leakage of ethanol, as well as pain and bleeding, superfine needles were used to inject ethanol for PEIT in the present study. In two patients with HCC in which the tumor lesions lay near the surface of the liver, a total of 14 doses of ethanol were injected for PEIT with 21 and 26 G needles, used alternately.
