A 60-year-old female had a history of high blood pressure for six years. Her abdominal echogram showed multiple hypoechoic lesions at the bilateral renal sinus which looked like hydronephrosis. Contrast enhanced computed tomography showed cystic lesions of water density in the bilateral renal sinus, suggesting parapelvic cysts. They were clearly differentiated from hydronephrosis. Thus, careful attention must be paid when evaluating the hypoechoic lesions at the sinus of the kidney with ultrasound.
