A case of congenital cystic adenomatoid malformation of the lung was diagnosed by ultrasound at the 23nd week of gestation. Ultrasonogram showed a 28×22×27 mm cystic lesion in the fetal thorax. The cyst was surrounded by a hyperechogenic area. The fetal heart was displaced slightly to the right sight of the thorax. The diaphragm was in the normal position and fetal ascites and maternal polyhydramnios were not present. Fetal thoracentesis under sonographic guidance was performed at the 23nd week of gestation and during labor. 5 ml of clear serous fluid was drained, but after the procedure, fluid in the cyst reaccumulated rapidly. A male infant with Apgar scores of 8 and 9 at 1 and 5 minutes was born at the 39nd week of gestation. The infant showed no evidence of respiratory distress. Because of future risks of pneumothorax and infection, a left lower lobectomy was performed on Day 47 and he was discharged on postoperative Day 17. Histologic sections were consistent with CCAML.
