Intussusception of adults is very difficult to diagnose pre-operatively. We report here an adult case of intussusception pre-operatively diagnosed through ultrasonography. An 81-year-old man was transferred to our hospital with a sudden onset of severe abdominal pain. On his second day at the hospital, ultrasonography revealed a severely dilated intestine wrapped within another intestine resembling an “intestine in intestine”, which enabled the immediate diagnosis of intussusception. Subsequently, we performed emergency surgery and found a jejuno-jejunal invagination with a lipoma. The sign, “intestine in intestine” seemed to be a definite diagnostic ultrasonographic finding of intussusception.
