Diastolic Mitral Valvular Function in Patients after Mitral Valve Replacement or Mitral Valve Repair: a Continuous wave Doppler Echocardiographic Study

The purpose of this study was to compare rest and exercise mitral flow dynamics in patients with a prosthetic mitral valve (Björk-Shiley valve) and in patients undergoing mitral valve repair with a rigid (Carpentier) or flexible (Duran) ring. A Doppler echocardiography was performed at rest and during exercise in 10 patients with a prosthetic mitral valve, 10 with a Carpentier ring and 10 with a Duran ring. There were no significant differences in the mean and peak transmitral flow velocities at rest among the patients who had received a mitral prosthesis, a Carpentier ring, or a Duran ring. However the mean and peak transmitral flow velocities at peak exercise were significantly higher in patients with a mitral prosthesis than in the patients with a Carpentier or a Duran ring. There were no significant differences in mean and peak transmitral flow velocities at rest between the two mitral valve repair groups; however, the flow velocities after exercise were significantly higher in patients with a Carpentier ring. In conclusion, patients after mitral valve repair show a better diastolic valvular function than patients with mitral prosthesis.
