Gastric fibroma is a rare benign neoplastic disease, that grows beneath gastric mucosa. Endoscopic ultrasonography (EUS) is an effective modality for studying internal images of submucosal tumors (SMT). In this report, we presented a case of gastric fibroma that was found in a upper gastro-intestinal X-ray. In the X-ray and regular endoscopic examination, the size of the tumor was 4.0×4.0×1.0 (cm) with delle on the top. EUS showed the gastric fibroma to be a hypoechoic solid tumor with a clear margin and with relatively homogeneous internal echo. Three normal layers of gastric wall were seen in front of the tumor, and five layers of gastric wall srounding the tumor were maintained normaly. No special features of the fibroma appeared in EUS, and it was very difficult to distinguish the fibroma as a myogenic or neurogenic tumor in this case. However, we suspected the fibroma of being benign based on criteria of EUS that have been demonstrated in myogenic SMT. In conclusion, clinically EUS was extremely useful for dealing with SMT including gastric fibroma.
