Two patients (56 and 72 years old) with chronic aortic stenoregurgitation presented with diastolic mitral regurgitation (MR). Pulsed Doppler echocardiography indicated an abnormal retrograde flow during mid-to late-diastole. Increased peak velocity without shortened deceleration in the rapid filling wave, and decreased peak velocity at atrial contraction in the left ventricular (LV) inflow were also present. Two-dimensional and M-mode echo-cardiographic examinations revealed dilatation, wall thickening and normal contraction of the LV. It is wellknown that diastolic MR often occurs in cases of acute aortic regurgitation (AR), but not in chronic AR. We think it worth reporting that our cases showed diastolic MR in spite of the presence of chronic AR.
