Primary malignant lymphoma of the spleen is a rare disorder. Only 10 previously reported cases with truly localized primary splenic lymphoma have been found in the Japanese literature. A 52-year-old man was admitted on August 1990 because of periodic physical check. A physical examination revealed no splenomegaly or systemic lymph-adenopathy. Laboratory data on admission showed slight hypersplenism. An abdominal ultrasonography revealed splenomegaly and multiple hypoechoic space-occupying lesions measuring up to 2.0 cm. The results of an ultrasound sonogram, computed tomography, 67Ga scintigram and angiography suggested splenic malignant lymphoma or hamartoma. A splenectomy was performed on January 1991. The obtained histological specimen revealed a malignant lymphoma (follicular type, mediumsized, B-cell type). 27 months after the splenectomy, a physical examination and image diagnosis revealed that there was no recurrence of the malignant lymphoma, and the case was diagnosed as one involving a truly localized primary malignant lymphoma of the spleen.
