A 41-year-old man was admitted to this institution for evaluation of a pancreatic tumor. A cystic mass freely communicating with the lumens of the portal, superior mesenteric, and splenic veins was found on ultrasonographic examination at time of admission and was diagnosed as an extrahepatic portal vein aneurysm. Magnetic resonance angiography (MR angiography) and Color Doppler ultrasonography confirmed the diagnosis and were also useful in investigating complications. Aneurysm of the portal vein, particularly that of the extrahepatic portal vein, is rare in Japan.
