To investigate the hemodynamic determinants of diastolic coronary flow, I used transesophageal Doppler echocardiography to analyze left ventricular pressure (LVP) and aortic pressure (AOP, measured with micromanometers), as well as flow velocity in the left anterior descending coronary artery (LAD). Eight of the subjects examined were normal (N); 7 had aortic regurgitation (AR); 14 had LV hypertrophy (LVH); and 3 had dilated cardiomyopathy (DCM). I calculated the time constant (Tc) of LVP decay and the gradient of AOP-LVP difference, the difference between the dicrotic notch of AOP and the lowest LVP (ΔP1/ΔT1), as well as between the lowest LVP and the end-diastole (ΔP2/ΔT2), and measured mean rates of acceleration (ACR) and deceleration (DCR), and peak velocity (PFV) of LAD flow. ACR was lower in DCM (2.5±0.6 m/sec2) and in LVH (2.9±0.8 m/sec2, p2). DCR was higher in AR (1.7±0.9 m/sec2) than it was in N (0.6±0.2 m/sec2, p1/ΔT1.
