Sonographic findings from 43 children with Epstein-Barr virus-induced infectious mononucleosis (EBV-IM) and results of biochemical liver function tests were compared. Lymphadenopathy was found around the liver hilum in 29 (67%) of the 43 cases, and was detected more frequently in infants than in school-aged children. Number of swollen lymph nodes correlated closely with severity of liver dysfunction. These results indicate that ultrasonography is useful in the early diagnosis of EBV-IM accompanied by liver dysfunction, especially in infants.
