Rupture of aneurysm in the aortic arch into the left pulmonary artery (PA) of a 77-year-old woman was demonstrated by transesophageal echocardiography (TEE). Conventional transthoracic echocardiography showed only aneurysmal dilatation of the aortic arch and a continuous flow signal in the PA. TEE, however, showed both the site of rupture as a continuity of the wall of the aneurysm and the left PA, and the continuous mosaic flow signal from the aortic aneurysm through the rupture site and into the left PA. Father, TEE also showed the aneur sm compressing the left PA just distal to the rupture site. TEE was thus shown to be useful in diagnosing rupture of an aortic aneur sm into the PA.
