Quadricuspid aortic valve is a rare congenital malformation that is much less common than bicuspid aortic valve. Thus it is often diagnosed in the operating room or at autopsy. The quadricuspid aortic valve has recently been identified by transthoracic and transesophageal echocardiography. We described a patient with severe aortic valve regurgitation in whom transthoracic and transesophageal echocardiography coupled with surgical findings confirmed the diagnosis of quadricuspid aortic valve. Brvsudr the aortic valve in this patient had four clearly formed cusps in both diastole and systole, a definitive diagnosis of quadricuspid aortic valve could be made on transthoracic and transesophageal echocardiography.
