Aortic arch vessel dissection associated with DeBakey type I aortic dissection diagnosed by two-dimensional and color Doppler echocardiography of the carotid arteries and confirmed at surgery is described here. Diagnosis of type I aortic dissection was made by transesophageal echocardiography (TEE). TEE did not reveal carotid artery dissection, although it detected involvement of dissection to the left subclavian artery. An additional approach using carotid artery echograms detected involvement of the bilateral carotid arteries. Carotid artery echography is useful in diagnosing involvement of the carotid artery, which is often difficult to visualize with TEE.
