We studied a five-month-old boy who had abnormal communication between the proximal and distal pulmonary arteries after pulmonary artery banding and in whom pulmonary hypertension and respiratory distress developed as a result of dilatation of the proximal pulmonary artery. Characteristic echocardiographic findings included band distally displaced close to the bifurcation of the pulmonary artery; proximal pulmonary artery significantly dilated; continuous-wave Doppler recordings showing peak velocity across the band to be decreased by half during the first two months after surgery, indicating reappearance of pulmonary hypertension; and color-flow mapping demonstrating two turbulent jets from the proximal to the distal pulmonary artery and the outside jet in the pulmonary artery showing the communication between the proximal and distal pulmonary arteries. These echocardiogrphic findings were confirmed on intracardiac-repair surgery. Two-dimensional echocardiography was thus useful in diagnosing this rare complication after pulmonary artery banding.
