Preoperative Ultrasonography in Patients with Primary Hyperparathyroidism

Eighty-three patients with primary hyperparathyroidism were examined preoperatively using 7.5 or 10 MHz real-time ultrasonography between 1989 and 1994. Seventy-four of these patients had a single adenoma, 2 had double adenomas, 6 had primary parathyroid hyperplasia, and 1 had a carcinoma. Ultrasonography correctly identified 71 of the 78 parathyroid adenomas, giving a sensitivity of 91%. Of the 25 glands in the 6 cases of hyperplasia, only 9 were identified, giving a sensitivity of 36%. One parathyroid carcinoma was correctly identified, and the overall sensitivity was 78% (81/104).
