肝腫瘍における造影カラードプラ法の検討

We attempted to access the usefulness of SH/TA-508, a new contrast agent, in improving color Doppler imaging (CDI) in hepatic tumors. We conducted a phase Ⅱ clinical trial using CDI in 18 patients with various hepatic tumors: 9 hepatocellular carcinomas (HCCs), 3 metastases (Mets), 3 hemangiomas (HEMs), 1 adenomatous hyperplasia (AH), 1 focal nodular hyperplasia (FNH), and 1 teratoma, using a color Doppler device (Toshiba SSA-260 A). CDI was performed before and after intravenous injection of SH/TA-508 (200 mg/ml, 300 mg/ml, and 400 mg/ml). Results obtained with enhanced CDI were compared with those obtained with conventional angiography and US angiography in 16 cases and those obtained with CT arterial portography in 9 cases. Nine HCCs, including one in which the tumor recurred after therapy, showed significant enhancement of flow signals after the agent was adiministered. One HEM showed gradual enhancement through the tumor margin, and 2 HEMs showed improvement of spotty flow signals. Three Mets showed tumor enhancement, and peripheral enhancement was shown in 2 of 3 cases. Flow signals showing a constant wave form in the precontrast study of one case of AH were enhanced in the enhanced Doppler study. One case of FNH with a tumor that measured 1.7 cm in diameter showed no flow signal within the nodule in the precontrast study; however, it produced a centrifugal flow pattern in the enhanced Doppler study. SH/TA-508 improves CDI and may enhance the demonstration of the characteristic hemodynamic pattern of hepatic tumors.
