We used tissue Doppler echocardiography, a new noninvasive modality, to evaluate left ventricular diastolic function. Conventional ultrasound equipment (Toshiba SSA-380 A) with software modification to display myocardial regional wall motion velocity by pulsed Doppler mode and color imaging was used. Sampling volume in the pulsed Doppler echocardiography used to measure myocardial wall velocity was placed at the middle of the left ventricular posterior wall. Thirty-nine normal subjects, 22 patients with left ventricular hypertrophy with normal ejection fraction (LVH/nEF), and 9 with impaired LV function (LV dys) other than myocardial infarction underwent quantitative pulsed Doppler examination of the posterior mid myocardium. Early diastolic peak velocity of the LV posterior wall (WmE) appeared after the LV isovolumic relaxation period and before peak mitral flow velocity. WmE was 14.4±4.0 cm/sec in the normal group, 10.5±3.1 cm/sec in the LVH/nEF group, and 9.4±2. 9 cm/sec in the LV dys group. WmE was significantly slower in the LVH/nEF and LV dys groups than in the normal group (P
