Noncompaction of the left ventricular myocardium is a rare disease that is characterized by numerous, prominent trabeculations and deep recesses in the left ventricle and is commonly associated with congenital heart diseases. Isolated noncompaction of the left ventricular myocardium (INVM) is rare and has been reported by pediatricians because prognosis was usually poor and patients were diagnosed during infancy or childhood. This disease is thought to result from abnormal cessation of the embryonic development of the ventricular myocardium. Thus few cases of INVM have been reported in adults. Here we describe two cases of INVM associated with severely disturbed left ventricular function and enlarged ventricle similar to dilated cardiomyopathy in a 65-year old woman and a 68-year-old man, both of whom had complaints of dyspnea. Their echocardiograms showed prominent trabeculations and deep intertrabecular recesses on the left ventricular walls, especially in the posterior and apex wall. Low ejection fraction and markedly enlarged and hypkinetic left ventricle were also demonstrated. Computed tomography showed marked trabeculations in the left ventricle. Patients with Previously reported cases of INVM ranged from 4 months to 44 years of age. Thus the two cases reported here are the oldest patients in whom INVM has been reported.
