A rare case of dilatation with the right ventricle and right atrium that rapidly deteriorated into right-sided silated cardiomyopathy (DCM)and was associated with progressive low voltage of the P wave is presented. Very few cases of DCM with low voltage of the P wave have ever been reported. This case suggests the possibility of a relationship between progression of P wave low voltage and progressive degeneration of right atrial smooth muscle.
