Portal Hemodynamic Investigation in Normal Volunteers by Color Doppler Velocity Profile (CDVP)

We used the recently developed color Doppler velocity profile (CDVP) technique to investigate portal hemodynamics in 35 normal volunteers. Visual inspection showed that the patterns of velocity profile in the portal trunk (PT), right anterior branch (RAB), and splenic vein (SV) changed over the cardiac cycle. A profile parameter n , equaled 1/(Vmax/Vmean-1), was used to determine the approximate shape of the velocity profile, and its values were 3.19±0.78 for PT, 2.70±0.53 for RAB, and 2.98±0. 81 for SV, indicating profile shapes between the parabolic and flat patterns. Quantitative evaluation of blood flow showed that maximum mean velocity and flow volume were 30.52±6.75 cm/s and 1062.7±152.3 ml/min, respectively, in the PTs; 12.82±2.69 cm/s and 415.5±136.4 ml/min, respectively, in the RABs; and 14.10±3.65 cm/s and 304.4±104.6 ml/min, respectively, in the SVs. Coefficients of variation for repeated measurement of flow volume were 8.38±4.07% in the PTs, 7.10±3.43% in the RABs, and 9.26±5.11% in the SVs. Distribution of flow volume over time (flow profile) showed lack of obvious undulation in 64.5% of the RABs and 59.3% of the SVs but changed periodically in accordance with cardiac cycles in 62.1% of the PTs, 35.5% of the RABs, and 40.7% of the SVs, and varied irregularly in 37.9% of the PTs. We conclude that CDVP can provide relatively accurate multivalent information about portal hemodynamic status. Moreover, it can utilize commercially available equipment and thus prove useful in clinical application.
