Several authors have recently used Doppler color flow mapping to observe split kidney function, and the use of this method for the accurate examination of split kidney function is becoming increasingly important. Thus changes in renal function should be examined periodically during treatment of hydronephrosis caused by stagnation of urinary flow resulting from obstruction by ureteral stone. Here we used Doppler color flow mapping to observe hemodynamic changes in split renal function. To confirm that renal function can be monitored in this way and further investigate this application, we used Doppler color flow mapping to study changes in renal hemodynamics in 45 consecutive patients with ureteral stones who underwent extracorporeal shock wave lithotripsy at this institution. We found that the renal hemodynamics of the treated side was significantly improved after treatment, while that of the contralateral side was not significantly changed. A higher grade of hydronephrosis on Ellenbogen's classification tends to further decrease renal hemodynamics. After treatment, however, renal hemodynamics improved to near the level of the contralateral kidney. Doppler color flow mapping may thus make it possible to monitor the recovery of renal function after treatment of ureteral stone. Additionally, it may provide information that could help in determining when treatment of the obstructed kidney is indicated.
