We performed transrectal prostatic biopsy under Doppler color flow imaging (CFI) in 164 cases during a period of 4 years and 4 months and studied the relation between biopsy results and vascular images of the prostate. Combination of more than three items from among the abnormal vascular images characteristic of malignant tumors, including localized vascular hyperplasia, irregularity in vessel diameter, diversity in direction of blood flow, presence of tortuous vessels, and presence of feeding vessels, was defined as tumor vascular image. Seventy-five (77%) of 98 cases in which cancer was detected by biopsy showed the tumor vascular image. Tumor vessel (-) was also found in 23 case, however. In prostatic cancer, as in renal cell carcinoma, the presence of a hypovascular tumor needs to be taken into account. At the same time, a new item, “absence of traveling of normal vessels”, should be added to the list of criteria suggestive of cancer. Of 66 cases in which cancer was not detected by biopsy, 57 (86%) cases showed tumor vessel (-). Of 9 cases in which tumor vessel was present in spite of the presence of cancer (-), 4 cases were diagnosed as inflammation.
