Transthoracic echocardiography performed on a 74-year-old male physician with a history of hypertension, atrial fibrillation, and silent cerebral infarction during a regular health checkup accidentally revealed a left atrial mass. The mass was 32×27 mm in diameter, and transesophageal echocardiography showed it to be attached by a stalk to the left atrial appendage. Chest CT examination also showed the mass in left atrium. The mass was surgically excised, and pathological examination of the resected material identified it as organized thrombus. Echocardiography is sensitive and specific in detecting intracardiac masses. These masses can usually be differentiated by their appearance, location, and site of attachment, Sometimes, however, it is difficult to differentiate a left atrial thrombus from a left atrial myxoma.
