We evaluated the usefulness of mitral annular systolic velocity derived from pulsed tissue Doppler imaging for assessing left ventricular ejection fraction (LVEF) in patients with an old myocardial infarction (OMI). Subjects consisted of 57 patients aged 66±10 years with OMI and 60 age-matched normal controls. Peak systolic (Sm) velocities were recorded at five sites on the mitral annulus corresponding to the septal, anterior, inferior, lateral, and posterior sites from the apical 4-chamber, 2- chamber, and long-axis views. The LVEF was calculated by modified biplane Simpson’s method. Sm velocities were reduced significantly more at all mitral sites in the OMI group than in the normal group, most profoundly at the infarcted sites. Comparison of Sm velocities of the anterior and inferior sites viewed from the apical 2-chamber view with LVEF≦45% showed that Sm velocities were lower in the inferior myocardial infarction group than in the anterior myocardial infarction group (p
