腹部超音波検査で診断された原発性虫垂重積症の1例を報告する．腹部違和感のある83歳の男性が虫垂部の腺腫の内視鏡治療を目的に当院を紹介された．腹部超音波検査では右下腹部にmultiple concentric ring signとhay-fork signを認め原発性虫垂重積症と判断した．hay-fork signの近位側には低エコーの腫瘤様所見を認め，虫垂の腫瘍が原因での原発性虫垂重積症と診断した．手術を施行し診断が確認された．腹部超音波検査は術前診断に非常に有用であった．

We reported a case of primary appendiceal intussusception diagnosed with abdominal ultrasonography. An 83-year-old man with abdominal discomfort was referred to our hospital for endoscopic resection of cecal adenoma. Abdominal ultrasonography depicted multiple concentric ring sign and hay-fork sign at the right lower abdomen, indicating appendiceal intussusception. And there was a low echoic mass at the proximal side of the hay-fork sign. We diagnosed primary appendiceal intussusception with appendiceal tumor. Surgery was performed and the diagnosis was confirmed. Ultrasonography was very useful in the preoperative diagnosis.
