外陰部の超音波検査には特有の困難がある．まず，プライベートパーツとも言われて，検者も被験者も，羞恥心がつきまとう．また，ドックなどでルーチンに行われるものではないので，正常所見を豊富に診る機会が限られる．したがって，経験・知識が不十分なうえに恥ずかしいので，さらに施行する機会が減る．その点を解消するには，系統だった知識と，精巣であれば健常側と丹念に比較することが重要となる．本稿では，外陰部，特に陰嚢内容の超音波検査の概説を行う．陰嚢内容腫大，急性陰嚢症，その他に分類し，シェーマを用いた概念的理解を促しながら，重要な所見と鑑別疾患に関して解説を行う．陰嚢内容腫大の項では，陰嚢水腫などの嚢胞性疾患と精巣腫瘍を中心に画像を提示する．急性陰嚢症の項では，精索捻転の診断と鑑別疾患の解説を行う．その他の項では，非触知精巣，外傷，その他の所見を提示する．

There are some difficulties associated with performing ultrasonography of the external genital organs, such as hesitancy. Moreover, ultrasonography of the external genital organs is not routinely done in health screening. Therefore, opportunities to perform ultrasonography of the external genital organs are limited for ultrasonographers, and even for urologists. A logical understanding of pathologies of the external genital organs and careful comparison between the affected and unaffected side are helpful to overcome these problems. In this article, ultrasonography of the external genital organs is reviewed, with intrascrotal swelling, acute scrotum, and other abnormalities discussed. Schema for easy understanding of each pathology and typical images are presented, including benign cystic lesions of the scrotum, testicular tumors, testicular torsion, impalpable testis, and testicular trauma.
