一次性下肢静脈瘤に対する治療の中心は伏在静脈逆流遮断であり，従来は外科的結紮，ストリッピングであったが，血管内焼灼術が臨床応用されるようになると，外科的治療より低侵襲に施行できることから，現在では治療法の第一選択になっている．この血管内焼灼術には周術期全般に血管エコーを必要とし，手術成績向上にも下肢静脈瘤の病態の理解に加え，血管エコー検査技術の習熟がカギになる．

The main target of treatment for primary varicose vein is the interruption of saphenous vein reflux. Currently, endovenous ablation is the first choice of saphenous vein interruption as it is less invasive than surgical ligation and striping, although surgical procedures were popular previously. Duplex ultrasound examination is essential for the procedure of endovenous ablation from pre to post operation. Learning how to perform a duplex ultrasound examination is the key to successfully performing endovenous ablation in addition to understanding the pathophysiology of varicose veins.
